Job Title: Integrated Case Manager
Department: Inpatient Case Management (Dept. 201247)
Location: 2799 W Grand Blvd, Detroit, MI 48202
Schedule: Days, 8:00 AM – 4:30 PM | Rotating Weekends

Key Responsibilities:
- Conduct comprehensive assessments of patient and family needs (biomedical, psychological, social, and functional).
- Develop and implement personalized, patient-centered care plans.
- Engage and educate patients and families, advocating for resources and services.
- Facilitate smooth transitions across care settings to ensure continuity.
- Collaborate closely with care teams on high-risk and complex cases.

Provide counseling and support around treatment decisions and end-of-life care.
Required Qualifications:
- Licensure:
- Registered Nurse (RN) – Active, unrestricted Michigan license
OR - Licensed Master Social Worker (LMSW) – Active, unrestricted Michigan license
- Registered Nurse (RN) – Active, unrestricted Michigan license
- Education:
- Nursing Degree (ADN or BSN)
OR - Master’s in Social Work (MSW)
- Nursing Degree (ADN or BSN)
- Certifications:
- BLS Certification through the American Heart Association (AHA)
- Experience:
- Minimum 5 years of recent Acute Inpatient Case Management experience
(Telephonic, therapy, or utilization management experience does not qualify) -
Proficiency in Epic EMR
- Minimum 5 years of recent Acute Inpatient Case Management experience
