Job Title: Integrated Case Manager
Department: Inpatient Case Management (Dept. 201247)
Location:  2799 W Grand Blvd, Detroit, MI 48202
Schedule: Days, 8:00 AM – 4:30 PM | Rotating Weekends

Key Responsibilities:

• Conduct comprehensive assessments of patient and family needs (biomedical, psychological, social, and functional).
• Develop and implement personalized, patient-centered care plans.
• Engage and educate patients and families, advocating for resources and services.
• Facilitate smooth transitions across care settings to ensure continuity.
• Collaborate closely with care teams on high-risk and complex cases.

• Provide counseling and support around treatment decisions and end-of-life care.

   

Required Qualifications:

• Licensure:
  • Registered Nurse (RN) – Active, unrestricted Michigan license
    OR
  • Licensed Master Social Worker (LMSW) – Active, unrestricted Michigan license
• Education:
  • Nursing Degree (ADN or BSN)
    OR
  • Master’s in Social Work (MSW)
• Certifications:
  • BLS Certification through the American Heart Association (AHA)
• Experience:
  • Minimum 5 years of recent Acute Inpatient Case Management experience
    (Telephonic, therapy, or utilization management experience does not qualify)

  • Proficiency in Epic EMR
