Job Summary:
The Supervisor, Community Prevention Services is responsible for overseeing daily operations related to HIV, STI, and HCV testing, education, and outreach programs. This role provides direct supervision to Community Health Specialists, ensures proper coordination of offsite events, and supports data collection and reporting efforts. The ideal candidate is a strong communicator and leader with experience in public health or harm reduction services and a commitment to advancing access to care within priority communities.
Duties/Responsibilities:
- Supervise Community Health Specialists, including coaching, scheduling, performance evaluation, and workload management.
- Lead and coordinate HIV, STI, and HCV outreach testing events across Trillium Health sites and community locations.
- Provide direct testing services and ensure clinical quality standards are met, including staff competencies and CLIA-waived testing procedures.
- Utilize electronic health record (EHR) systems to ensure complete and accurate documentation; EPIC experience .
- Monitor and analyze program data; prepare internal reports and support external reporting requirements.
- Manage inventory and procurement of testing supplies and outreach materials.
- Collaborate with internal departments and external partners to improve access to care and strengthen community relationships.
- Contribute to the design and delivery of prevention and health education initiatives.
- Ensure compliance with public health guidelines, organizational policies, and quality improvement standards.
- Participate in professional development and stay informed on emerging trends in community health and harm reduction.
Required Skills/Abilities:
- Must be able to communicate effectively in English, both verbally and in writing.
- Proven ability to supervise and support a team in a community health or outreach setting.
- Strong understanding of HIV, STI, and HCV prevention, testing, and linkage to care.
- Knowledge of harm reduction principles and ability to apply them in practice.
- Proficient in documenting services and managing data using electronic health record (EHR) systems; EPIC experience a plus.
- Strong organizational and time management skills, with the ability to manage multiple priorities.
- Skilled in building partnerships and representing programs in community and public health settings.
- Comfortable providing direct services, including testing, education, and counseling.
- Ability to work independently and collaboratively in a fast-paced, mission-driven environment.
Education and Experience:
- High school diploma or GED required.
- Bachelor’s degree in Nursing, Health & Human Services, Public Health, or a related field strongly ; equivalent experience will be considered.
- Minimum of 2 years of supervisory or team leadership experience required.
- At least 3 years of experience in community health, health education, prevention services, or outreach required.
- Prior experience providing HIV, STI, or HCV testing and counseling strongly .
- Knowledge of harm reduction practices and experience working with priority populations .
- Experience using electronic health record (EHR) systems required; EPIC experience a plus.
- Bilingual in Spanish and/or American Sign (ASL) strongly .
Physical Requirements:
While performing the duties of this job, the employee is regularly required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms; and talk and hear. The employee may occasionally need to stoop, bend, and lift or move up to 25 pounds. Specific vision requirements include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
