Reports To: Client Care Manager / Director of Care
Location: Colorado (Field-Based)
Status: Part-Time
Schedule: Weekends (On-Call Rotation); Field visits as needed
Compensation: Hourly and shift differential for weekend on-call
Position Summary:
Talem Home Care is seeking a compassionate and reliable Certified Nursing Assistant (CNA) to serve in a field supervisory role on a part-time basis, including participation in a weekend on-call rotation. This position plays a key role in ensuring quality of care, supporting caregivers in the field, and assisting with client well-being checks. The ideal candidate will have strong communication skills, sound judgment, and a passion for home-based care.
Key Responsibilities:
- Conduct supervisory visits to client homes to observe and support caregivers, ensuring care plans are being followed and client needs are met.
- Provide field-based coaching and feedback to caregivers to improve service quality and adherence to agency standards.
- Communicate with clients and their families to ensure satisfaction and address any concerns or changes in condition.
- Participate in a rotating on-call weekend schedule, responding to caregiver call-outs, urgent care needs, or client concerns.
- Document all supervisory visits and communications clearly in the electronic health record (EHR) or designated system.
- Collaborate with the administrative team to identify care plan changes, report incidents, or escalate concerns.
- Assist in onboarding and for new caregivers when scheduled on weekends.
Qualifications:
- Active CNA license in good standing with the State of Colorado.
- At least 1 year of experience in home care, assisted living, or skilled nursing settings.
- Valid drivers license, reliable vehicle, and proof of auto insurance.
- Comfortable working independently and making field decisions with support from management as needed.
- Excellent interpersonal, communication, and documentation skills.
- Availability to work weekends and share responsibility in an on-call rotation (typically every 24 weeks depending on team size).
Physical Requirements include but are not limited to:
- Able to stand, kneel, bend at knees, and squat throughout the day.
- Ability to safely assist clients in and out of a vehicle and safely lift a wheelchair or walker weighing up to 50lbs into and out of a vehicle.
- Physical ability to lift and transfer clients
- Use arms, legs, feet, hands, and fingers to assist clients with performing tasks of daily living.
- Frequently move the whole body to perform tasks such as lifting, light housekeeping, walking, and driving.
- Sitting for long periods of time
- Ability to see and hear well (naturally or with correction)

- Prior supervisory or team leadership experience is a plus.
- Experience using EHR or scheduling systems.
- CPR certification.
Compensation:
