Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. The center is dedicated to delivering high-quality, culturally competent services that enhance the quality of life. Position Summary:


Responsible for operation of direct support services programs. The major programs are healthcare case management, grant funded case management, food and nutrition, and support groups.



Job Responsibilities:

  • Ensure overall compliance of programs assigned under the department of support services at Queens location such as service deliverable level, quality, budget, and reports.
  • Assist the Associate and Assistant Director of Support services with grants management including to create the assigned projects under Support Services Department on revenue projection and expense budget.
  • Recruit and hire program staff assigned.
  • Provide clinical and administrative supervision to staff by chart review, face to face case discussion and performance reviews.
  • Actively participate in multidisciplinary meeting.
  • Develop and execute recruitment and enrollment plan to increase client base for Apicha.
  • Support and manage staff to meet quality and quantitative requirements.
  • Perform quality assurance and quality improvement activities.
  • Maintain and update educational and promotional materials.
  • Troubleshoot any crisis with clients, medical providers, and mental health providers.
  • Work closely with other programs and departments in the organization to ensure a high quality comprehensive continuum of services.
  • Submit both external and internal reports in a timely fashion.
  • Implementing and providing Support Services related training to the staff.
  • Actively participate in any organizational activities (meetings, events, etc.).
  • Represent the agency at various venues to promote agency services.
  • Performs other activities as assigned.

Qualifications:

  • BA/BS relevant field with 5 years full time experience in relevant field required. May consider advanced degree in social work, human services, mental health or equivalent with 3 years of full-time experience.
  • Two (2) years of supervisory experience to paid staff .
  • Experience working with target populations strongly . The target populations: individuals with Serious Mental Illness, Children and family, developmental disabilities, serious substance use disorders (SUDs), chronic hepatitis, LGBT, high risk of or living with HIV/AIDS and/or Asians and Pacific Islanders.
  • Appreciation of and commitment to Apicha’s mission and values.
  • UAS-NY certification required within 3 months of hire.
  • Excellent customer service skills to assist in resolving any issues.
  • Ability to work in multi-disciplinary environment and diverse staff.
  • Effective and professional oral and written communication in English required.
  • Familiarity with managing government funded contracts .
  • Ability to perform public speaking and making presentations in a professional manner representing Apicha Community Health Center.
  • Ability to work effectively in both a collaborative as well as individual setting.
  • Ability to use and adapt to various applications (Outlook, Word, Excel, PowerPoint, eCW, eICARE, AZARA, HCS, Healthix, MAPP, PSYCKES, etc) required.
  • Ability to multitask and manage multiple projects in a stressful environment is strongly recommended.
  • Ability to work flexible hours including some evenings and weekends.
