As Program Manager, you will be responsible for supporting the success of contracted partners (care coordination and case management services providers, and health-related social need (HRSN) benefit providers). The Program Manager is responsible for the Community Care Hub service agreements and contracts between Elevate Health and its care coordination and case management partners, and HRSN benefit providers. This role provides program onboarding of contracted partners, program implementation and support, contracts monitoring, partner support and coaching for continuous quality improvement, and technical assistance and training in collaboration with the Community Care Hub Operations Manager. This is a highly collaborative role and will support Community Care Hub and organization-wide projects to promote community-based care coordination best practices and quality outcomes throughout the provider network.


Heres what youll be doing:

Partner Support:

  • Serve as primary point of contact for Community Care Hub contract partners including onboarding, training, liaising, and weekly, monthly meetings with each partner.
  • Develop and ensure access to, updated training, resources, and support for Community Care Hub contract partners.
  • Provide coaching and program technical assistance to contract partners related to documentation of services, records-keeping, billing, reports, and deliverables outlined in service contracts and agreements.
  • Collaborate with Elevate Health team members in identifying and connecting partners to community resources and training to support contract partners including best practice learning opportunities.
  • Establish and maintain feedback systems with community stakeholders including, but not limited to network partners, Pierce County and those with lived experience, and past participants and partners to ensure:
    • Meaningful and relevant services connection for Pierce County
    • Monitoring of program performance.
    • Contractual obligations with funders and service partners.
    • Support quality assurance and improvement.

Program and Community Care Hub Network Management:

  • Provide programmatic oversight, reviews and monitoring/auditing of care coordination workflows and client records completed by community-based health workers to ensure compliance and best practice.
  • Partner Engagement
    • Foster relationships with Community Care Hub and Social Care Network partners to support and enhance services delivery.
    • Field new partner interest requests and make recommendations to senior management and leadership in alignment with community needs and Community Care Hub requirements.
    • Identify and respond to gaps in services and care coordination/case management needs of community.
    • Provide peer-to-peer networking, support, and training opportunities to contract partners.
    • Provide input on programs and department budgeting to leadership.

Contract Monitoring:

  • Develop measures, audit tools for contracted service providers, and evaluate performance and outcomes in collaboration with interdepartmental team members.
  • Facilitate meetings with various community stakeholders for feedback.
  • Ensure partner participation in evaluations and continuous quality improvement activities.
  • Implement Plan-Do-Study-Act processes and improve or resolve identified issues.
  • Communicates regularly with other managers, leadership within the organization, and other designated contacts in the community.
  • Engage in the contracting process of Community Care Hub partners in collaboration with Community and Clinical Linkages staff and Finance staff.
  • Support contract initiations, renewals, and closings with Community Care Hub network partners and HRSN benefit providers.
  • Review and provide program-level approvals of contract partners billings and reports.

  • Adhere to HIPAA security and compliance standards in all work.

Staff Management:

  • May supervise and support program staff, including recruitment, training, and performance evaluation, to foster a high-performing team environment.
  • Coordinate staff schedules and resource allocation to meet service demands.

Data Management & Reporting:

  • Utilize data analytics to track the performance of contracted partners and community impact.
  • Prepare comprehensive reports on program performance and metrics for partners, stakeholders, and leadership.
  • Implement data-informed decision-making to optimize operational efficiency.

Quality Assurance:

  • Define regulatory standards and best practices in community care
  • Identify and lead quality improvement initiatives to enhance service delivery and client experience
  • Works to improve community-based care coordination processes, communication, and client care as it relates to health-related social needs.


Heres what were looking for:

  • Bachelors degree in Community Health, Public health, social services, non-profit management, social, or related field (masters degree ) AND
  • Minimum of 5 years of experience in program and operations management within community health or related sectors; OR
  • Any combination of education, professional, volunteer and lived experience that provides the necessary knowledge, skills, and abilities to perform the duties and responsibilities.
  • Experience working in communities addressing social and health care disparities.
  • Experience providing social or clinical coordination services in Pierce County .
