The Program Manager serves as the primary leader of the residential support team and is responsible for developing and overseeing the implementation of Individual Support Plans.
Essential Duties and Responsibilities Include the Following: (Other duties may be assigned.)
- Reviews assessments to support the person in identifying the supports and services needed to address their goals and desired outcomes.
- Participates in shared planning by assisting each person in identifying and communicating their requests and needs for services and support.
- Monitors support to ensure that each persons goals and desired outcomes are being addressed based on assessments, interviews, and observations. .
- Makes modifications and adjustments to the ISP by soliciting the persons feedback.
- Reports progress, significant changes/events and modifications/adjustments to Support Coordinators quarterly by completing reviews, determining appropriateness of services, and by recommending changes as appropriate.
- Maintains the Comprehensive and Service Documentation records for each person supported.
- Oversees many aspects of group home/supported living activity including appearance of home, leisure materials, cleanliness and community integration.
- Spends time on a regular basis at the location where services are being performed. to ensure policy, procedures and program data are in place and being implemented appropriately.
- Expected to provide shift coverage in times of acute shortages of other direct support staff but continues to be responsible for the primary management duties.
- Trains staff on new and changing programs to ensure correct and timely implementation.
- Works closely with other clinical/professionals.
- Participates in conducting formal and informal investigations into incidents/allegations of abuse, neglect, exploitation or other circumstances that may present risk to safety and health.
- Reviews incident reports to ensure treatment was provided, cause of injury determined, and preventative measures implemented.
- Performs various assessments, chart reviews, medication administration observations, mealtime, environmental and other monitoring.
- Works closely with the Administrator and Site Managers regarding employee work schedules, disciplinary actions, job performance and personal conduct.
- Attends quality management team meetings.
- Facilitates relationships between the company, parents/guardians and Support Coordinators.
- Obtains consents as needed.
- Stays updated on Cornerstone policies and procedures, service manuals and other regulations.
- Drafts responses to and implements changes brought about by quality assurance audits and surveys and/or other regulatory agencies.
- Seeks to maintain all services in compliance with all regulations without deficiencies.
- Carries out supervisory responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include interviewing, hiring, training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Ensures confidentiality regarding sensitive material including employee and service users individual rights to privacy, and protected health information.

- Performs other duties as assigned.
- Work requires a willingness to work a flexible schedule and travel.
- Practices universal medical precautions by understanding and utilizing personal protective and safety equipment.

- Shall have experience in working with the population served and in providing the services outlined in the Service Description.
- Shall maintain state and company training requirements to work with individuals with developmental disabilities.
- Ability to supervise Site Managers and Direct Support Staff.
- Ability to develop and deliver presentations.
- Ability to conduct training for personnel.
- Work requires professional written and verbal communication and interpersonal skills. Ability to communicate and interact with officials at all levels of government and to work effectively with a wide range of constituencies in a diverse community.
- Ability to motivate teams and simultaneously manage several projects.
- This is normally acquired through a combination of the completion of a bachelors degree in a Human Services field or equivalent experience and experience of working with the population served.
Physical Demands
- Working conditions are normal for an office environment.
- Work may require frequent weekend and evening work.
