GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

Perform the duties required to develop, implement and sustain a coordinated chronic disease care model in HIV infected patients. Coordinate one-on-one health education. Supervises and assists other Case Managers. Serves as their time-card supervisor in the HR software system. Administers and follows policies and procedures approved by the Shenandoah Community Health Board of Directors.

Clinical

  • To be responsible for intake assessments that includes patient history of present health status, vital signs, current medications, allergies, and significant previous medical/psycho-social problems.
  • To educate patients on HIV testing, compliance, treatment options, risk reduction, and prevention.
  • To follow standing orders regarding medication refills, specific laboratory tests, and updating immunizations for HIV patients.
  • To maintain supplies and educational material.
  • Periodically compile and report quality data as required to maintain grant eligibility.

Organizational

  • Develop and maintain patient access to interagency health supportive services by participating in community initiatives and collaborative activities with other community agencies.
  • Identify patient needs and refer them to appropriate internal and external support services.
  • Participation in procuring and monitoring grants with HIV activities.
  • Budget input and tracking – coordination with accounting for input on client costs.

Community

  • Collaborate with community agencies to increase awareness of HIV care.
  • Develop partnerships with regional resources to provide necessary supportive services for HIV patients.

Clinical Information Systems

  • Assist in design and implementation of computer tracking templates for various programs
  • Formulate statistical and narrative reports.
  • Communicate regularly with Department of Health Administration and HIV/AIDS, Hepatitis, STD &TB Administration to ensure integrity of the program.
  • Periodic audit of CAREWare to assure accurate data entry.
  • Attend and present at teleconferences with DC Health and other associated organizations.

Delivery System Design

  • Assist in design and implementation of patient focused delivery model.
  • Interacts with providers to ensure comprehensive care for the patient.
  • Re-enforce program goals and objectives for HIV case management and patient education.
  • Attend staff and department meetings as needed.

Decision Support

  • Work collaboratively with the providers in the development and implementation of clinical practices guidelines for HIV management.
  • Plan and coordinate staff in-services regarding HIV risk reduction.

QUALIFICATIONS:

  • Must be a Certified Medical Assistant or Licensed Practical Nurse
  • Ability to function effectively within a multidisciplinary team.
  • Skills in patient communication and education.
  • Annual basic CPR certification.
