Job DescriptionJob DescriptionRole Summary: Position will provide Intra-Departmental support for accurate and timely flow of client information and database training and accuracy. Facilitator will receive and coordinate requests from all departments requesting client information and respond promptly with results. Position will field and assist clients with questions concerning program and detox procedures directing them to pertinent staff or outside resources.Role Requirements and Preferences:

Required:

  • High School diploma or equivalent OR 2 years’ experience in a similar role.
  • Minimum 6 months mentor/supervisory experience (Will be overseeing Peer Mentors.)
  • Completion of Onboarding within the first month of hire, which consists of completing a review of the policy and procedures manual, the employee handbook, and review of benefits.

  • Familiarity with administrative functions and tools (customer service, MS Suite, physical office management, etc)
  • Familiarity with the Healing Transitions service model and the recovery process.

Core Responsibilities:

  • Administrative Support: Provide intra-departmental support to ensure the accurate and timely flow of client information and database maintenance accuracy.
  • Collaborate closely with the Director of Recovery Services, Planning Room, and Peer Mentors to manage daily tasks including the review of all program documentation to ensure accuracy, including daily program rosters, bed assignments, participant attendance, and arrivals or departures.
  • Work in partnership with Recovery Engagement Specialist staff to maintain a consistent flow of participant progress, movement updates, and roster changes.
  • Assist with facilitation of Community Process Business Meetings for the CTR I Community.
  • Collaborate with Peer Mentors to establish and maintain consistent operations within the Planning Room. Support ongoing growth within the Planning Room by role modeling and providing to Peer Mentors.
  • Proactively meet with the Planning Room Coordinator and Director of Recovery Services to discuss potential improvements to systems, policies, and procedures to enhance efficiency and service quality at Healing Transitions.
  • Required to attend weekly program coordination meetings to provide overviews and updates of pertinent information regarding program participants.
  • Build and maintain community partner relationships such as probation/parole, mental health providers, Trillium Health Resources.
  • Document Management: Manage and organize documents, ensuring all client information is accurately recorded and easily accessible.
  • Ensure that all documentation requiring the Planning Room Coordinator’s review or approval (such as pre-screen forms, program changes, release of information, and program discharges) is correctly and accurately prepared.
  • Manage the TextBehind account to ensure timely distribution of correspondence to inmates from program participants.
  • Communication: Receive and coordinate requests from all departments for client information, responding promptly with results. Field and assist clients with questions concerning program and detox procedures, directing them to pertinent staff or outside resources.
  • Respond promptly and accurately to Planning Room emails and voicemails, addressing or following up on related inquiries. Ensure Peer Mentors are also promptly and accurately responding to emails and voicemails.
  • Compliance and Confidentiality: Adhere to all compliance regulations and maintain confidentiality of client information.
  • Miscellaneous:. Handle other duties as assigned by the supervisor.
