CloseKnit is currently hiring a DMV (D.C., Maryland, Virgina) based hybrid registered nurse (RN) to join our team as a Clinical Health Navigator. We are an advanced primary care practice supporting in person and virtual care across all 50 states, committed to providing high-quality, team-based care that integrates seamlessly with our patients' lives. Our model prioritizes clinical excellence, patient engagement, and innovative care delivery. In addition to primary care, we lead a advocacy program that helps patients navigate the health care system and access the care they need. We are seeking a dedicated Clinical Health Navigator (RN) to join our team and make a meaningful impact.
Role Overview
As a Clinical Health Navigator, you will play a critical role in delivering patient-centered care and clinical guidance across our hybrid primary care practice and advocacy program. This position requires a registered nurse (RN) with strong clinical knowledge, excellent communication skills, and the ability to provide compassionate support to patients navigating their healthcare journey. You will collaborate with internal and external providers, medical assistants, and health guides to address patient needs, support complex care coordination, and deliver outstanding service.
Key Responsibilities
Clinical Triage, Navigation, & Patient Support:
- Serve as the first point of contact for symptomatic patients providing virtual and/or in person clinical triage in a fast-paced, critical-thinking environment for urgent and complex care needs in line with clinical validated and recognized Nursing Standards of Care.
- Provide timely responses to non- clinically urgent patient inquiries, leveraging evidence-based guidelines.
- Support patients with chronic conditions, ensuring they understand their care plans and have access to necessary resources.
- Answer questions about lab results, medication management, and health conditions, and escalating issues to providers when necessary.
Care Coordination & Patient Advocacy:
- Facilitate care transitions, including referrals to specialists, behavioral health providers and employer point solutions.
- Curate and maintain a panel of patients under the supervision of clinical leadership coordinating services for patients receiving care in the ED/Hospital and coordinate discharge planning
- Provide longitudinal care planning to a panel of patients by creating and maintaining a care plan in coordination with their PCP to ensure patients meet both their personal and clinically validated health care goals.
- Help advocacy clients understand their healthcare benefits and access appropriate care and point solutions available to them through their employers.
- Collaborate with internal and external provider teams to address social determinants of health impacting patient outcomes.
Collaboration & Team Integration:
- Work closely with health guides, medical assistants, and providers to create a seamless care experience for patients.
- Act as a liaison between providers and patients to ensure clinical concerns are addressed promptly.
- Participate in workflow optimization and quality improvement initiatives to continuously improve the care we provide for our patients.
Documentation & Compliance:
- Accurately document all patient interactions in the electronic medical record (EMR) system and our customer support platform.
- Ensure compliance with all federal, state, and organizational regulations, including HIPAA and patient safety standards.
Qualifications
Required:
- Active and unrestricted compact RN Licenses
- Minimum of 3+ years of clinical experience in primary care, care coordination, or case management.
- Experience in virtual care delivery or telehealth.
- Strong understanding of chronic disease management and preventive care.
- Proficiency with electronic medical record systems and healthcare technology.
- Exceptional communication and interpersonal skills.
- Proficiency in Microsoft Office use.
- Proficiency in working with electronic medical record systems.
- Proficiency working with technology and across multiple systems.

- Experience in healthcare advocacy or working with payer/provider organizations.
- Knowledge of social determinants of health and experience addressing health inequities.
- Certification in care management or related specialties (e.g., CCM, CPHQ).
