Client Care Manager

As aClient Care ManageratCommunity Connection Programs, Inc., youll lead a team of compassionate caregivers delivering home care services to seniors and individuals with disabilities. Your role ensures clients receive exceptional, compliant, and person-centered care while supporting caregivers with the tools and guidance they need to succeed.


What Youll Do

Supervise & Support Caregivers

  • Lead and coach a team of in-home caregivers
  • Conduct performance evaluations and provide ongoing training
  • Schedule and assign caregivers to meet client needs

Monitor Client Care

  • Conduct home visits to ensure quality and compliance
  • Collaborate on and update personalized care plans
  • Quickly address concerns from clients or families

Ensure Compliance & Quality

  • Maintain accurate care records, progress notes, and incident reports
  • Audit documentation to meet state regulations and agency standards
  • Participate in quality assurance activities and surveys

Be a Reliable Point of Contact

  • Respond to emergencies and provide on-call support
  • Ensure caregivers are trained in emergency procedures

Collaborate & Communicate

  • Partner with the Regional Program Director and admin team
  • Coordinate with scheduling to ensure proper coverage


Qualifications

  • Bachelors degree () in healthcare, social work, or related field
  • Experience in home care, healthcare, or team supervision
  • Knowledge of home care regulations and documentation best practices
  • Strong leadership, communication, and critical thinking skills
  • Passion for helping others and improving lives
  • Reliable transportation & valid drivers license
