JOB SUMMARY

The Program coordinator will assist with the daily coordination of individuals care needs in compliance with Medicaid and DBHDS program rules and regulations. The Program Coordinator is responsible for providing day-to-day managerial oversight of our DD In-Home Support, Community Engagement and Community Coaching Services in Region 1 and Region 2 service areas as structured by DBHDS.

JOB DUTIES

This position is responsible for employee supervision, retention, training, and development. In addition to the organization and implementation of direct support, person-centered planning, advocacy, regulatory compliance, safety, and maintenance of the physical environment.

Guarantee adherence to licensing, certification, and regulatory protocols for all services within the designated caseload. Schedule and conduct regular quality assurance assessments.
Supervise program staff members, encompassing performance evaluations, scheduling, and onboarding processes.
Offer comprehensive supervision and active monitoring of behavioral programming, ensuring the implementation of the least restrictive interventions, upholding dignity and individual rights, and employing effective behavioral support strategies. Develop and maintain a dedicated staff team focusing on Individual Support Plan (ISP) and Behavioral Support Plans (BSP) outcomes.

Oversee person served involvement in the community, home environment, person-centered planning, and the promotion of individual choice. Uphold program structure and the rights of individuals served through continual support, individual supervision, training, and frequent staff meetings and Support day-to-day client advocacy.

Duties to include:

  • Maintain and coordinate a caseload of clients enrolled in R.A. Quarshie Healthcare programs.
  • Conduct necessary monthly client home visits and assessments of service provisions by staff.
  • Coordinate clients in-home support and community Service needs in compliance with Medicaid and DBHDS rules and Regulations and requirements.
  • Assist with intake processes to ensure positive business growth and development.
  • Participate in client ISP meetings and develop accurate and complete meeting reports to higher-up.
  • DSP staff supervision.
  • Ability to meet position requirements with little direct oversight
  • Collaborate with stakeholders regarding the delivery of services
  • Effective written and verbal communication
  • Comply with applicable legal requirements, standards, policies, and procedures
  • Handle complex service programs
  • Maintain necessary skills and knowledge to coordinate workflow
  • Participate in professional development and training activities
  • Represent the company at stakeholder meetings, health fairs, and provider fairs as needed
  • Other duties as assigned

QUALIFICATIONS

  • Bachelors degree in the field of Human Services, Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field
  • QDDP a plus
  • 3 years or more previous managerial experience working in the field of human services preferably working with individuals with developmental and intellectual disabilities.
  • Demonstrate dependability
  • Demonstrate effective problem-solving and decision-making skills
  • Exhibit computer efficiency
  • Experience with WAMs system a plus
  • Experience with Therap System a plus
  • Bilingual a plus
  • Ability to prioritize and multitask effectively
