Care Coordinator II Compensation

• The pay for this position is $23 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on one of our eligibility dates

Care Coordinator II Highlights

• Remote – Kentucky only (Eastern or Western KY)
• Training: First 2 weeks, 8:30 AM – 5:00 PM EST, cameras on
• Schedule: M–F, 8:30 AM – 5:00 PM EST

Care Coordinator II Responsibilities

• Conduct outbound calls to Medicare members (~10 calls/hour)
• Complete assessments and document interactions accurately
• Educate members on preventive care and address social determinants of health (SDoH) barriers
• Schedule appointments with providers to close open care gaps
• Navigate multiple systems while maintaining member engagement
• Participate in weekly office hours and monthly 1:1s for guidance
• Utilize internal resources to resolve member questions

Care Coordinator II Requirements

• High School Diploma or GED
• 1–2 years of related experience (call center or provider office environment)
• Proficient in Microsoft Excel and Word
• Knowledge of medical terminology
• Strong verbal and written communication skills
• Ability to work independently while managing multiple systems
• Compliance with all company policies and standards

Care Coordinator II Preferences

• Previous experience in healthcare or Medicare member outreach
• Familiarity with HEDIS measures and care gap closure processes
