Case Planner opportunity available in Brooklyn!
One of our clients, a large, non-profit organization, is seeking Case Planners for their Family Foster Care Division. This position will be starting asap on a long term temporary basis with potential for temp-to-direct hire. The hours are Monday - Friday, 9am - 5pm with potential for over time. This role will require home visits throughout Brooklyn, Manhattan and surrounding areas, as well as court appearances.
Responsibilities will include, but not limited to:
• Provide direct case planning services to children in the foster care program
• Maintain mandated contacts, develop service plans and connect children and families to appropriate resources
• Document progress notes and accurate reports
• Make home visits and court appearances, and liaise with ACS and OCFS
• Facilitate and actively participate service planning, reviews and family conferences
Qualifications:
Bachelor's Degree
Experience in child welfare, or working with children/juveniles
Computer proficiency in MS Office and ability to quickly learn other database systems
Bilingual Fluency in Spanish or Creole strongly .
