Job DescriptionPrimary Care Physician (Hybrid, On-site & Remote) - Colorado Springs, CO
Schedule: Full-Time | Monday-Friday | Hybrid On-site & Remote
Pay Range: $220k - $250k Annual

Overview: We're seeking a compassionate Primary Care Physician to join our SNF Transitions Team, delivering subacute and palliative care to geriatric patients. Ideal for physicians with hospitalist or SNF experience seeking a meaningful role with excellent work-life balance.

Responsibilities:

• Provide subacute/palliative care in SNF
• Lead care planning and IDT meetings
• Guide goals-of-care and advance care planning

Requirements:

• BE/BC Physician, CO license; active DEA
• 2+ years hospitalist/SNF/post-acute experience is required
• Strong in geriatrics, chronic disease, and palliative care
• Bilingual is a plus
